REGISTERING YOUR LUCKY DOG IS EASY!

If you can’t open and print these blank forms they are available at Lucky Dog.

Step 1:

Make sure your dog meets the criteria for the class for which you are registering. Update your dog’s vaccines if they are not current. Please write the name of the trainer,
and location of the class (Lucky Dog Central or South) on top of the enrollment form.

Step 2:

Step 3:

Print enrollment form                     GO FETCH registration form! paw

Step 4:

Bring your completed forms,
a copy of dog’s shot records and your class fee to Lucky Dog as soon as possible to reserve your class space. You may also snail mail your paperwork but remember, all classes are filled on a “first-come, first-served” basis. If you are signing up for private lessons you will be contacted with date, time and location.
